HOMELINK and U.S. Rehab
HUMANA ANNOUNCEMENT

This contract covers complex rehab and mobility equipment and services, and will be available to Humana members covered under most of their plans including ChoiceCare/Commercial and Medicare Advantage plans

Due to unique pricing requirements with the Humana contract, U.S. Rehab and HOMELINK are requiring providers to sign an agreement for participation in the program.  Providers who sign this agreement will have the opportunity to gain incremental business by marketing their complex rehab and mobility services to local referrals sources.   

Reimbursement Details

Reimbursement will be according to the HOMELINK/Humana fee schedule minus deductible and/or co-insurance responsibility.  Pricing is non-negotiable.

Terms and Conditions

  • All Humana referrals are MANDATED AND REQUIRED to have the Functional Mobility Assessment (FMA) Outcomes Tool administered. Once you pay your EULA fee, U.S. Rehab will contact you to setup FMA training for your team.  
  • Providers must agree to the reimbursement rates associated with this contract.
  • Providers must complete HOMELINK’s credentialing requirements.
  • Providers must submit claims to HOMELINK within 30 days of the date of service.
  • Provider must participate and be in good standing with Medicare.

How to submit your referrals to HOMELINK

Working with HOMELINK is convenient and easy.  Give us a call, fax, email, or electronic request.  A HOMELINK associate will be ready to help.

HOMELINK will issue payment to providers within 7 days of receipt of payment from Humana.
